Who was first?
It's about the first albums of the style that we consider today as symphonic black metal
not about those ablums where symphonic elements appeared giving rise to a later genre.

Necrodictator, Nope
Grzechu, I think it's Arcturus «My Angel EP» from 1991. Before that some stuff from Mortem (pre-Arcturus), but it's more death metal-related. Honestly, I didn't remember stuff from «Slow Death» very good.

An interesting challenge, a return to the past
When the term was first used in the music press, who did it, for which band etc
Investigation in progress

Necrodictator, Someone think he so smart. But I think this is misunderstanding. The author of the question probably meant the release, not LP specifically.
EP it's mini-album by the way. Author don't say full-lenght album. So?

According to wikipedia, the first band using keys has been Bulldozer's "Neurodeliri" back in 1988... though they never did it again on their next releases, nor they can be labelled as Black Metal, like Arcturus. So Emperor "In the Nightside Eclipse" was probably the starting point

Master's Hammer - Ritual 91
It's hard to say, it's hard to define the boundary to which the band has symphonic elements in music
and which bases the entire composition on a classical/symphonic composition.

Master's Hammer - Ritual 91
I listen album long ago, but I don't think that he has a place to be here. Try to listen it right now.
Necrodictator's «
Jilemnický okultista» from
Master's Hammer sounds more correct for our topic.
By the way, «
Ritual» are great.

If we take the topic's title quite literally than the first album featuring symphonic black metal would be Bulldozer Neurodeliri. The title song is genuine symphonic black metal even by modern standards.
